ACCESS TO INTERACTIVE WEB-BASED VERSIONS
The only web-based electronic versions of our questionnaires are the ones available through this site and supplied by us. This is to ensure that every questionnaire in every language is accurate, gives valid data and you can have confidence that you are using an authorised and tested instrument. All languages have been checked by the MAPI Research Institute.
Methods of Access
We provide two ways to access our web based questionnaires.
1. Direct Access (click here for a comprehensive description)
This is the simplest method and the one used most frequently by clinicians and academics but it can also be used by pharmaceutical companies for clinical trials and by other commercial enterprises.
In brief, we provide you with a username and password to our questionnaire website. After going to the log-on page on this website, you will be asked to enter your username and password. You can then select the required questionnaires which will appear on the screen where they can be completed by the patient. As soon as the patient has responded to all the questions the results, as either raw data or as a PDF file of responses and summary scores, can be saved to your computer for you to store or print.
We do not and will not store any patient data on our web-site. We only retain the date and time you accessed the questionnaire. All patient information is erased from our site either as soon as you should log off or close the browser and your session expires.
2. Integrated Access (Click here for comprehensive description and technical documentation)
This method may be used by those who wish to integrate one or more of our questionnaires into their own web-based electronic data collection system. Users are mainly pharmaceutical companies who have electronic case records for their clinical trials and large clinical organisations.
In simple terms, we provide a web service that displays the questionnaires in the correct format and returns the results. For example, in a clinical trial, when a patient attends the clinic, the clinician will log onto the pharmaceutical company's web-site and access that patient's case record form. The clinician will work systematically through each of the tasks required at that visit. At the point when our questionnaire has to be completed by the patient, the pharmaceutical company's web-site will automatically call up our web-site, with the correct authorisation and the questionnaire will appear on the clinician's screen. Immediately the patient has completed the questionnaire, all data will be sent to the pharmaceutical company's web-site and the clinician will move on to the next task.
Like the direct method of access, we do not store any patient data. As soon as the patient has completed the questionnaire, the data are sent to the user's server and we retain only the time and date of access. If an audit trail is required by regulatory agencies (e.g. ID number), this can be arranged but we will not hold anything that might be considered confidential.
USER FEE STRUCTURE
For all users there is an annual registration fee which is based on the number of questionnaires required and the number of languages. Once you have paid the registration fee, you may use the questionnaire as many times as you like for the next 12 months.
For clinicians and academics who are planing to use a questionnaire in their own clinic for the management of their own patients or non-commercially funded research, the fee is nominal (it covers the amount we pay our web provider for providing the clinician's/ academic's access).
For commercial organisations, the annual fee is slightly higher in order to cover our own running costs and support our ongoing research and development program (e.g. validation of the electronic versions of our questionnaires on this site and support of cultural adaptations of our questionnaires for countries where commercial and research funds are not available).

HOW THE PROCESS WORKS
When we receive your submission, we will contact you, either by e-mail or phone, to discuss the type of access that you would like to use (direct or integrated) and the registration fee for the questionnaires you wish to use.
After identifying your needs, we will ask you for a purchase order number and the person and address to whom the invoice is to be sent. As soon as we receive your purchase order, we will issue you with your username and your password.
When you receive your password, you will be able to access and use the questionnaire/s you have requested as many times as you like during the next 12 months. There is no restriction on the number of times you may access the site.
RENEWAL OF REGISTRATION
At 9 months, you will be sent an e-mail asking if you would like to renew your registration fee for another year. If you do not respond immediately, we will repeat the message at 10 and 11 months and thereafter at weekly intervals. If we have not heard from you at 12 months, we will assume that you do not wish to re-register and your password will cease to be active.
The renewal will come into effect at the end of your previous 12 months registration period (i.e. you do not lose out by renewing your registration in plenty of time!)
